The above programs offer a wide range of outpatient services to the culturally diverse adult population of the south region in San Diego County. The emphasis of our treatment is on the psycho-social rehabilitation of the individual. Respect for the client is a core value, and our staff work closely with all of our clients to develop individualized service plans that focus on their strengths, needs and abilities. General Description
The Licensed Geriatric Outreach Specialist provides home visits, screenings, assessments, short-term counseling, crisis intervention, case management, and outreach activities within a psychosocial rehabilitation treatment model to Older Adult consumers (ages 60 years and over). Service will be provided across the South Region in coordination with both MSCWRC and SBGWRC. Assists clients in accessing and using medical care, community resources, family and other support systems to achieve an optimal level of health and functioning in the community. Linkage, outreach, and consultation to other service providers for the older adult population are also required.
Qualification/Skills Requirement
Must be a licensed RN, LMFT, LCSW, Ph.D. or Psy.D. Bilingual Spanish required. Preferred applicants: experience working with Older Adults who have Serious and Persistent Mental Illness. Basic computer skills are also required. A valid State of California driver's license in good standing and auto insurance coverage within 10 days of employment is required and must be maintained during course of employment.
